Event Support Acknowledgment

By submitting this form, I understand that my payment or contribution is being provided in support of The Great Trunk or Treat, hosted by Rough Cut Jeep Club.

I understand that all event support payments and contributions are final and non-refundable. Proceeds from this event support Rough Cut Jeep Club operations and future community outreach projects.

I understand that Rough Cut Jeep Club is not representing this contribution as a tax-deductible charitable donation, and no tax-deductibility is promised or implied.

By providing a business/organization name or logo, I give Rough Cut Jeep Club permission to use the submitted name and/or logo for event-related recognition, promotional materials, social media, signage, and acknowledgments associated with The Great Trunk or Treat.

Event support does not create a partnership, endorsement, ownership interest, or other business relationship between the supporter and Rough Cut Jeep Club, Louisville Metro Government, Louisville Parks and Recreation, Councilwoman Crystal Bast, or other event supporters.

Participant Acknowledgment & WaiverBy signing below, I acknowledge that participation in The Great Trunk or Treat is voluntary and at my own risk. I understand and agree that Rough Cut Jeep Club, Louisville Metro Government, Louisville Parks and Recreation, Councilwoman Crystal Bast, event organizers, volunteers, and supporters are not responsible for loss, theft, or damage to vehicles or personal property, or for personal injury that may occur while participating in or attending the event at Sun Valley Park, to the fullest extent permitted by law.I understand that I am responsible for my vehicle, personal property, decorations, equipment, and conduct while participating in the event.All registration fees, event support payments, and other monetary contributions are final and non-refundable. By signing below, I acknowledge that I have read, understand, and agree to the terms above.
